CONGENITAL DIAPHRAGMATIC HERNIA

Patients with diaphragmatic hernia will be managed with careful attention paid to possible need for ECMO. However, not all CDH patients require ECMO, and for that reason they will be managed at St. Paul Children's until the above stated ECMO criteria are met. If the patient is failing on conventional ventilation, consideration will be given to high frequency ventilation, either High Frequency Oscillation or Jet Ventilation. Surgery is routinely delayed for several days until the issue for possible need for ECMO is resolved. For this reason the Surgeons will be notified, expecting that surgery would not be done urgently; if it is a night admission, discuss with the Neonatology attending whether or not Surgery needs to be notified at that time.
