Lungs: good air entry, some bibasilar crackles

Cardiovascular: regular rhythm and rate, grade III/VI systolic murmur, loudest over LLSB, weak peripheral pulses, moderate central pulses

Abdomen: soft, liver edge palpable 4cm below right costal margin, and spleen tip is palpable
