Pre-adoption Consultation Submission Form

Please make sure to complete and send a submission form to the IAC if you plan on sending materials for review during your trip, even if your agency will be facilitating communication with us.

Things to look for when meeting your prospective child

The Denver Developmental Pre-screening Questionnaire

Denver Developmental forms can be used to gather medical information on your prospective child.  The questionnaire is not available for download; however, you may request a hard copy, or copies, from the IAC.  Please e-mail or call our business office at 612-624-1164 to request materials.

Please indicate the child's age:

• 0-9 mo.
• 9-24 mo.
• 2-4 yrs.
• 4-6 yrs.
